DECLARATION BY THE TRAINEE
1. I affirm that all the information provided in the application form are true, correct and
complete to the best of my knowledge.
2. I shall abide by the rules and regulations of the institution.
3. On being granted admission, I also agree to pay all the fees and other charges as per
the schedule framed by the institute.
4. I understand that I am liable for removal from the institution for indiscipline or
misconduct either inside or outside the institution.
5. I am responsible for my own fault, if necessary the Principal can take necessary
action.
6. I shall attend my classes regularly and also pay my fee failing which I shall be
debarred from attending classes.
7. Fee once paid will not be refunded or adjusted in any case.
8. I am ready to pay separate charges for extra facilities provided by the institution,
which is beneficial for me.
9. I do here by agree that I will not claim for any refund under any circumstances.
10. Smoking. Foodstuff & Alcohol are strictly not allowed in the training car & campus.
11. SAI-DTS, BARGARH Management is the final Authority to establish the new rules and
regulation of the institution.

DECLARATION BY THE GUARDIAN
I do hereby declare that, my Son / Daughter / Wife________________________________
is an applicant for admission in SAI DRIVING TRAINING SCHOOL (SAI-DTS), BARGARH, if admitted will abide by the rules and regulations of the institution. I have no objection if he / she will be expelled / driven out from the institute, if found guilty any reson whatsoever.
